The pledge card (bid form) has been removed pending a better copy... the one I had was far too ugly (bad scan) to use for such a formal purpose.

A copy of the form to order supplies from the National Business Manager is available here. Please print out a copy and mail the filled out form and payment to the address listed on the form.
